We encourage medical facilities to contact our intake manager to schedule a chart review and patient interview. This requires a signed prescription for Home Health that details the patient's diagnosis and needs.
Patients and Relatives
- Your doctor faxes the home health order to us.
- Our intake department gathers the necessary information.
- Once insurance is verified, our intake team will call you.
- A nurse will then follow up with you to begin care.
