Our Medicare Care Process


Medical Professionals

We encourage medical facilities to contact our intake manager to schedule a chart review and patient interview. This requires a signed prescription for Home Health that details the patient's diagnosis and needs.

Patients and Relatives

  • Your doctor faxes the home health order to us.
  • Our intake department gathers the necessary information.
  • Once insurance is verified, our intake team will call you.
  • A nurse will then follow up with you to begin care.

Our Other Services

Beyond Medical Care


While our registered nurses provide expert medical care, we understand that overall well-being depends on more than clinical needs. Through our services, clients also have access to support with Activities of Daily Living (ADLs) provided by Certified Nursing Assistants (CNAs) or Home Health Aides, ensuring comprehensive care.

This non-medical support includes:
